As Ann attends exorcism classes, she faces resistance from the staff and ridicule from some priests. Disturbing events plague her, including haunting visions, suppressed memories, and an unsettling encounter with a possessed elderly patient who traps her in his room. Undeterred, Ann dives deeper into her training, uncovering hidden archives with Dante’s help. These records reveal "terminal cases" expected to die during possession and the mysterious story of a woman who was released from the institute.
Ann successfully exorcises Dante’s sister but is devastated to learn the woman took her own life shortly after. Overwhelmed with guilt, Ann resolves to return to her convent. Before she can leave, Dante informs her that Natalie, who had briefly recovered, has relapsed and is now classified as a "terminal case," destined to be sent to the Vatican. Determined to help, Ann sneaks back into the school with Dante to perform an unauthorized exorcism.
During the ritual, Ann discovers a shocking truth—Natalie is her biological daughter, whom she had given up for adoption as a teenager. The abandonment had left Natalie vulnerable to demonic possession. Summoning extraordinary strength, Ann confronts the demon, which ultimately leaves Natalie and possesses Ann instead. Ann’s victory earns her an academic fellowship to the Vatican. However, as she rides in a cab, she realizes the driver is the elderly man who previously harassed her under demonic influence. The vehicle halts, and Ann spots a sinister woman—the same individual released from the institute—watching her from the street. Clutching her crucifix, Ann braces herself as the driver lunges at her, revealing that her battle against evil is far from over.
